plus.html is the HTML form page containing text input fields and two buttons. The text input fields are labelled:

• Username: which contains the text hr
• Password: which contains asterisks
• Connection String: which contains the text Ora9i
• Report to run: which contains the filename employee.sql.

The two buttons are labelled:
