Import the LDAP Password ActiveSync Form into Identity Manager.
This form is defined in $WSHOME/sample/forms/LDAPPasswordActiveSyncForm.xml.

In the Active Sync wizard for the resource, set the input form
to LDAP Password ActiveSync Form.
