The Instructions for Self Service 1095C Statement self-service page appears when employees click the ACA 1095C Statement link in their Employee Self-Service responsibility.
As an employee, you can:
Provide and track explicit consent to receive the 1095-C statement electronically.
View the notifications required by the IRS publication.
Obtain the instructions that belong with the year's 1095-C statement.
The following sections explain the contents of the ACA 1095C Statement page.
The initial 1095-C Statement page provides information about the process to obtain the 1095-C statement electronically. The instructions are related to providing the consent, revoking consent, and the procedure for obtaining a paper copy after giving consent to receive electronically.
Before getting access to the 1095-C statement, employees must explicitly elect to receive the form electronically.
If you want to receive the 1095-C statement electronically, then select the Consent check box and click Save to record your consent. When you click Save, the Search Archive Data page appears.
Click Revoke, if you want to withdraw your consent to receive the 1095-C statement electronically and save your changes.
The application stores a history of consents and withdrawals with the date on which the action is performed. On termination, a person's consent is canceled so that they can receive the statement as a paper copy as an ex-employee.
After confirming consent and clicking Next, if there are no 1095-C statements, then the application displays a message that are no statements that are available for viewing. If 1095-C statements are available, then the Search Archive Data page appears.
In the Search Archive Data page, to find the 1095-C statement that you need to view, search using one or more of the following criteria:
Reporting years that have active viewing as of the system date.
GREs for which the forms are archived.
Version numbers of statements available for the reporting year and GRE selected.
If there is more than one version available for that reporting year and GRE, then an additional search drop-down is available to select the version number. The search defaults to the latest version.

Part II Employee offer and coverage. For individuals who are offered Individual Coverage Health Reimbursement Arrangements (HRAs), Part II displays the age as of the first of the year and the Line 17 zip codes.
Part III Covered individuals
Click View Latest PDF to view your 1095-C statement using the seeded laser-cut template for the applicable reporting year. You can print or download the 1095-C statement.
The page also displays a link that you can click to access the instructions for the year for which you are viewing the form.
