CRF Status by Visit report

Summary

For each patient visit, this report displays the total number of system and manual form statuses.

A subtotal appears for each patient. A total row appears at the bottom of the table.

Charts and graphs

• Bar graph—Displays the number of forms in the following form statuses for each visit:
  • Not Started
  • In-Progress
  • Open Query
  • Complete
• Pie charts—Display the following data for each patient:
  • Percentage of forms with the status Not Started.
  • Percentage of forms with the status In-Progress.
  • Percentage of forms with the status Open Query.
  • Percentage of forms with the status Complete.

Column

Description

Site Number

Identification number assigned to the site.

Site Name

Full name of the site.

Patient ID

Unique identifier for the patient.

Visit Name

Name of the visit as displayed in the user interface.

Total Forms

Total number of forms in the Not Started, Incomplete, Incomplete with Queries, and Complete statuses.

Not Started

Total number of forms in the visit with the status Not Started.

In-Progress

Total number of forms with the status In-Progress.

Open Query

Total number of forms with the status Open Query.

Complete

Total number of forms in the visit with the status Complete.

Remote Reviewed

Total number of forms in the visit with the status Remote Reviewed.

DM Reviewed

Total number of forms in the visit with the status DM Reviewed.

Source Verified

Total number of forms in the visit with the status Source Verified.

Signed

Total number of forms in the visit with the status Signed.

Frozen

Total number of forms in the visit with the status Frozen.

Locked

Total number of forms in the visit with the status Locked.
