CRF Status by Form report

Summary

For each patient, this report displays the system and manual form statuses of each form. A subtotal appears for each visit. A total row appears at the bottom of the table.

Charts and graphs

For each of the following statuses, a pie chart displays the percentage of all forms in that status represented by each form in the study:

• Not Started
• Open Query
• Complete

• Patient ID
• Visit Name

Column

Description

Site Number

Identification number assigned to the site.

Site Name

Full name of the site.

Patient ID

Unique identifier for the patient.

Visit Name

Name of the visit as displayed in the user interface.

Form Name

Name of the form.

Not Started

Yes or blank, indicating whether the form status is Not Started.

The report shows subtotal counts of forms in this status by visit, patient, and site and a total count of forms in this status for the study.

In-Progress

Yes or blank, indicating whether the form status is In-Progress.

The report shows subtotal counts of forms in this status by visit, patient, and site and a total count of forms in this status for the study.

Open Query

Yes or blank, indicating whether the form status is Open Query.

The report shows subtotal counts of forms in this status by visit, patient, and site and a total count of forms in this status for the study.

Complete

Yes or blank, indicating whether the form status is Complete.

The report shows subtotal counts of forms in this status by visit, patient, and site and a total count of forms in this status for the study.

Remote Reviewed

Yes or blank, indicating whether the form status is Remote Reviewed.

The report shows subtotal counts of forms in this status by visit, patient, and site and a total count of forms in this status for the study.

DM Reviewed

Yes or blank, indicating whether the form status is DM Reviewed.

The report shows subtotal counts of forms in this status by visit, patient, and site and a total count of forms in this status for the study.

Source Verified

Yes or blank, indicating whether the form status is Source Verified.

The report shows subtotal counts of forms in this status by visit, patient, and site and a total count of forms in this status for the study.

Signed

Yes or blank, indicating whether the form status is Signed.

Frozen
