Viewing the audit trail for a visit or form
The audit trails for visits and forms are accessible to users at all times. The Audit Trail button appears on every page. You can also view the audit trail covering changes to the visit and form data from the selected form.
- Select Patients (
).The Patient List page appears.
- Click the Summary link for the patient.
The Patient Summary page appears.
To view the audit trail for a visit:
- Click a visit.
Note: A visit that has not been started; that is, has the
icon in the Status section, has no values in the audit trail.
- On the Visit Landing page or the form, click the Audit Trail button (
).The Audit Trail page for a visit appears.
- To see the changes made to visit information, click the plus sign to the left of the item in the Field column.
or
To see all changes, click the Show all details link at the top of the page.
For each item the most recent change appears first.

- Click Back.
To view the audit trail for a form:
- Expand a visit to view the forms within the visit.
- Click a form.
The OutcomeLogix application displays the selected form.
- Click the Audit Trail button (
).The Audit Trail page for a form appears.
- To see the changes made to form information, click the plus sign to the left of the item in the Field column.
or
To see all changes, click the Show all details link at the top of the page.
For each item the most recent change appears first.
