The Site Visit Report page is a form that allows you to prepare a site visit report. The format of this page, which is an optional feature of the InForm application, is customizable for each study and is created when your study is set up.

Site | Site associated with the subject. Click the link to open the report. |
Date | Date of the report. |
Site filter | Drop-down list of all the sites you can access. |
