Add or remove item from a form—For study version and in-place revision changes

When a form is affected by a study version or in-place revision change, the form state is always recalculated regardless of whether the change includes a new Partial SV setting.

If the change includes a new Partial SV setting (for example, the criticality is changed), the new form state is applied to all study versions.
