Verify that the Originator (identified by the Origin-Host AVP in the message) is generating Diameter Requests with either User-Name AVP or MSISDN AVP being present.
Inspect each element between teh GQC and GQS to determine which node is inserting bot AVPs and correct that node so that only one AVP is included in the GGR.
