Description: An abbreviation that identifies a site, as entered in the InForm Administration interface.

Address 1

Data type: String

Description: First line of the site address, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Address 2

Data type: String

Description: Second line of the site address, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

City

Data type: String

Description: The city in which a site is located, as entered in the InForm Administration interface.

State/Province

Data type: String

Description: The state or province in which the site is located, as entered in the InForm Administration interface.

Country

Description: The country in which a site is located, as entered in the InForm Administration interface.

Postal Code

Data type: String

Description: The postal code for the site address, as entered in the InForm Administration interface.

Phone

Data type: String

Description: The primary telephone number for the study site, as entered in the InForm Administration interface.

Alt Phone

Data type: String

Description: The site secondary telephone number, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Fax

Data type: String

Description: The fax number for a study site, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Email

Data type: String

Description: The email address for a site contact, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Time Zone

Data type: String

Description: The time zone in which a study site is located, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Date Format

Data type: Numeric

Description: The format used for all references to dates for the site. Valid values are:

• 1—Day/Month/Year
• 2—Year/Month/Day
• 3—Month/Day/Year

Site Activation Date

Data type: Date/Time

Description: The projected date for the activation of the study by the site. It does not necessarily reflect the actual date on which the activation occurs.

Date type: May be specified in study design code. If not, this is a system date.

Current Study Version

Data type: String

Description: Displays a description of the most current study version, as specified in the study design code. Many study designers use this field to indicate study version iteration. For example, 2 indicates the second version of the study. The value may also be a text description for the study version.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Current Site Acceptance Date

Data type: Date/Time

Description: The date on which the site was assigned to the current study version.

Date type: May be specified in study design code. If not, this is a system date. The date that appears here depends on how the site was associated with the current study version:

• If the site was associated with the study version through the InForm Administration interface, the Current Site Acceptance Date is the system date when the association took place.
• If the site was associated with the study version through the MedML Installer utility, the Current Site Acceptance Date is the date specified in the MedML for the new study version.
