Subjects: Subject Status Dates

The Subjects: Subject Status Dates report elements return dates when subjects were assigned to particular states in the InForm application.

Subjects may be set to some states more than one time. For these states, First and Last date report elements appear in the InForm Ad Hoc Reporting tree. Use these report elements to identify when subjects are moved from one state to another, as follows:

• Use Last dates to determine the current status for a subject and the date on which the status change occurred.
• Use the First and Last dates together to identify subjects who were, but are no longer, in a given state.

  Report element	Description
  Subject Status Date	Data type: Date/Time Description: The date on which the subject was assigned the current subject status. For example, if the current subject state is Complete, the Subject Status date indicates the date on which the subject status was set to Complete. Date type: System date (GMT).
  InForm Screened Date	Data type: Date/Time Description: The date on which the subject was screened for the study in the InForm application. A Screened status indicates that the Screening form for the subject was started, but the subject is not yet enrolled. This covers these cases: The subject is fully screened, but the enrollment form has not been started. The subject is fully screened and the enrollment process has started but not completed. Data type: Clinical
  InForm Enrolled Date	Data type: Date/Time Description: The date on which the subject was enrolled in the InForm application. A status of enrolled indicates the subject was properly enrolled in the InForm application, but has not reached the status of randomized, completed, or dropped out. Date type: System date (GMT).
  InForm Screen Failure Date	Data type: Date/Time Description: The date on which the subject failed the screening process. Date type: System date (GMT).
  InForm Randomized Date	Data type: Date/Time Description: The date on which the subject was randomized through the InForm application. A status of randomized indicates that a subject was enrolled and randomized in the InForm application, but is not yet completed or dropped out. Note: If you perform randomization outside of the InForm application, the resulting report column is empty. Date type: System date (GMT).
  First Dropped Date	Data type: Date/Time Description: The date on which the subject first dropped out of the study, as recorded in the Study Completion form for that subject. Date type: System date (GMT).
  Last Dropped Date	Data type: Date/Time Description: The date on which the subject most recently dropped out of the study, as recorded in the Study Completion form for that subject. If the subject is removed from the dropped out state, the resulting report column is empty. Date type: System date (GMT).
  Subject First Complete Date	Data type: Date/Time Description: The first date on which the subject completed the study, as recorded in the Study Completion form as designed per InForm requirements. Date type: System date (GMT).
  Subject Last Complete Date	Data type: Date/Time Description: The most recent date on which the subject completed the study, as recorded in the Study Completion form as designed per InForm requirements. If the subject is removed from the Complete state, the resulting report column is empty. Date type: System date (GMT).
  CRB SV Ready Date	Data type: Date/Time Description: The date on which the case book was marked SV ready. Date type: System date (GMT).
  CRB Frozen Date	Data type: Date/Time Description: The date on which the case book was frozen in the InForm application. If the CRB is unfrozen, the resulting report column is empty. Date type: System date (GMT).
  CRB Signed Date	Data type: Date/Time Description: The date on which the case book was signed in the InForm application. If the CRB is not signed, the resulting report column is empty. Date type: System date (GMT).
  CRB Locked Date	Data type: Date/Time Description: The date on which the case book was locked in the InForm application. If the CRB is not locked, the resulting report column is empty. Date type: System date (GMT).
