•         Street Address (Lines 1-4)

•         Apartment/Suite

•         Country Code

•         Postal Code

•         City 

•         State/Province Code

•         Email Address

•         Email Opt-In Status

•         Phone Number: This field displays only if it has been selected for display in the Work with Contact Center (WWCC) menu option.

•         Mail (Receive Catalog Mailings)

•         Rent (Share Information with Other Companies)

•         Customer Class
