The top area of the page has the following initial fields and values:
Service Request = Get_AddressBooks
Product Code = 55 (selected from a drop-down list)
Short description text box = Get_AddressBooks
A Long Description button

Next are fields and options for loading the data items fields from which you want to return data, which include:
Table/View Name = V0101B (with the business view name displayed next to it - "Address Book-One Line Report")
Load button
Get View from Form button
Aggregation toggle (disabled)

Next is the Fields grid, which displays the following column headings:
Description - displays the name of the field in the table or business view.
Data Dictionary - displays the name of the data item for the field in the Data Dictionary.
The rest of the columns display buttons for performing actions for each field displayed in the grid, which include:
Filter
Return
Aggregate
Having
Group By
Order By

The grid is displaying the first nine controls for the V0101B business view, which a vertical scroll bar for viewing the remaining controls in this business view.

Adjacent to the Fields grid is an area for configuring conditions for the selected filter fields and return fields. The following controls are directly above this area:
Data Set Variable Name field
Options button
Preview button

The first section in this area contains the following options for defining filtering criteria:
Match Type
Match All
Match Any
The "'$' incidicates variable" informational text displayed next to these options.

Below these options is a Conditions box which contains a single condition, which is Address Number [F0101] followed by a field for selecting an operand, which contains > (greater than symbol), followed by a field for entering a value, which contains 0. 

Below the Conditions box is a section entitled "Return Fields and Variable Names," which displays all the fields that were selected as return fields. Next to each field is a field for entering a variable for the field. This section contains the following fields; none have variables defined:
