In the upper left corner is the Custom List title. Below this are the Save list, Cancel button, Reset button, Change ID button, and Actions list. Below this is the Name field, ID field, Internal ID field, and Owner list. Below this are the Values tab (which is selected), Translation tab, History tab, and System Notes tab. Below this is a table with the Values column (which includes the values E-mail, Mail, No advertisements, and Phone), Translation column, and Internal ID column (which includes the values 2, 3, 1, and 4).
To the right of the Name field is the Description field, the Show Options In selection (with options for The Order Entered and Alphabetical Order, the Convert to Custom Record check box, and the Inactive checkbox.
