This is the Instance page from which you can select the service level for Oracle Java Cloud instance. 
For this tutorial you will select Oracle Java Cloud with Fusion Middleware, along with Oracle Data Integrator option. Bear in mind that you will be charged for provisioning the Fusion Middleware stack as it is not part of the standard installation.
The page consists of a button, Cancel, at the top left and another button, Next, at the top right.  Below are two columns of text fields and drop-down lists.
The first text field in the left column is Instance Name, populated with MyJCSODI.  This field is mandatory.
Below is the Description text field, which is optional to populate.  The third field in the column is Notification Email.  This is optional, and is populated with the user e-mail address.  
Finally is a drop-down list called Tags.  It is left blank, and is optional.
