At the top is the "What is it called" section, which includes fields for Name and Identifier. Below this is the "What does it do" section, which includes fields for the following:

• Description
• Document Standard (X12 is selected)
• Document Version (4010 is selected)
• Document Type (850 (Purchase Order) is selected)

At the lower right are Cancel and Create buttons.
