From top to bottom, the dialog box has the following information:

• Select from Previous Presentation drop-down list. The selected value is Employees.
• Name field. Its value is Approvers.
• Description field. This field is empty.
• Make default check box.
• Switch to this presentation check box. This check box is selected.

The Create and Cancel buttons are in the lower-right corner.
