This image shows the Add Type Definition panel. The fields and the values entered in each field are as follows:

1. Name: Applicant Data
2. Mode: Complex
3. isList? check box: Left unchecked.
4. Define Type Attributes: This section has three attributes defined within a table. The table contains four columns : Name, Type, Value/Values, and is List. The values in each row corresponding to these columns are as follows:
   • Name, Text, Text, false
   • Phone, Number, Number, false
   • Account Number, Number, Number, false

There's a Close button at the bottom of the dialog.
