The first row of the decision table contains the Decision Table label to the left and controls to add or delete rows or columns to the right. From left to right, controls available are: Expand Table, Add Rule Before, Add Rule After, Remove Rule, Add Column Before, Add Column After, and Remove Column. The second row contains the Hit Policy Indicator cell that has the Unique hit policy selected, a first Input Expression cell that has the input variable Salaried used as the input expression, a second Input Expression cell that has the input variable ExistingCustomer used as the input expression, a first Output Label cell named BaseRate, a second Output Label cell named MaxTenure, and an Annotations cell. There is an Allowed Values cell within each Input Expression cell and Output Label cell. The value for this cell is set as true, false for the Input Expression cells and left blank for the Output Label cells. There are four subsequent rows that constitute rules of the decision table. Each row consists of the following: a cell indicating the rule number (starting from 1 to 4), two Input Entry cells in the Input Expression columns, and two Output Entry cells in the Output Label columns, and an Annotation entry cell in the Annotations column.
