This screenshot shows the dialog box for creating a new template. The dialog box contains:

• Type checkbox. Choose Field or Form.
• Label. Enter the name of the template.
• ID. This is generated based on the Label field.
• Description. Enter a description of the template.
• Enable Extensions. App extension will be able to reference this template
• Create and Cancel buttons.
