Failure Capture for Work Order Assets
Capture failure data for a maintenance work order asset to record what failed, why it failed, and how it was resolved. This information supports reliability analysis, maintenance effectiveness, and future maintenance planning.
Set up failure capture
Before users can capture failure data:
- Create failure, cause, and resolution codes using Condition Event Codes.
- Create at least one failure set. A failure set is required for failure
capture and can:
- Limit users to valid failure, cause, and resolution event code combinations.
- Make failure capture optional or required for associated work orders.
- Associate the failure set to the appropriate asset, item, item category, or define it as the default.
Define a failure set
A failure set defines the rules and applicability for failure capture. Begin by defining a unique combination of name, code and optionally a description.
Then, consider the Use chains for failure capture option. By default, this option is set to Yes, requiring that you define one or more failure chains, which control the combination of failure codes you can select. You can optionally set it to No, allowing you to select any combinations of failure codes without defining any failure chains. A failure chain is a valid combination of failure, cause, and resolution codes for an asset or group of assets. Define one or more chains to guide users to relevant code combinations and improve consistency. After defining the chains, you can deactivate them as required.
Next, define at least one association for a failure set. Associations are used by the work order to derive a single applicable failure set that can be used for capture. When defining a set, first select the level of applicability.
- Asset: Used when the failure set is defined for a specific asset. For an asset type association, asset number and description fields are displayed.
- Item: Used when the failure set is defined for assets associated with certain item number. For an item type association, the master organization, item number, and description fields are displayed.
- Item Category: Used when the failure set is defined for assets associated with certain item category associated with asset item number. For an item category type association, the category name and description are displayed, and you must select the item’s master organization.
- Default: This is a generic failure set that's used as default during failure capture when none of the other failure association exists for the work order asset.
You can only have one active failure set with an active default association at the time. If you select another set as being the default, it presents a click through confirmation that inactivates the currently defaulted set.
The level of applicability is used to validate the appropriate plan, following the lowest level first:
- Asset type matching work order asset.
- Item type matching item associated to the work order asset.
- Item category type matching item category associated to the work order asset item.
Finally, for each association, define if the failure set is required.

If a higher-level set is mandatory, but the lowest level isn't, then the failure set always takes the direction from the lowest level of applicability.
Care should be taken when requiring a failure set always or only for Corrective work orders, because the failure set is required for every matching work order and prevents work order completion until the failure event and at least one report is captured.
After failure sets are created, you can edit them and their details, but you can’t end-date them. Deactivating associations controls the applicability within work orders.
Capture failure data
You can capture failure data for an open maintenance work order using the Report Work page in My Maintenance Work. Begin by creating a Failure Event, selecting the date and an optional indication of the asset is down for maintenance or not. You'll see the appliable failure set that was automatically derived but can’t be updated. Only one event can be created for a work order.
After an event is created, you can create one or many individual failure reports, based on the failure set. For each report, you must select the Failure, Cause and Resolution code, based on the option Use chains for failure capture. You can optionally select the asset or component item that failed, applicable operational set and describe the failure. You can record multiple failure entries for a work order asset and update the information until the work order is closed
