Before you start 
 The Manage user-defined forms to display and collect data
permission is required.
   
 
Here's what to do 
- Click a user-defined form.
- Add a text field to the form by positioning your cursor
in a cell and clicking the Insert Text icon to place a text area onto
the form. 
- Select the field you just added and scroll to the Selected Element Properties section at the bottom. 
- Place the cursor in the Value text field and click the
Enlarged View icon.
- In the Enlarged View page, click the Insert or edit image
icon.
- Enter the URL of the image location.
 Enter image property information, if desired. 
 
- Click OK.
- Click Save.
