• The Cancel button.
• The Register button.
• The Endpoint Name field, which is required.
• The Type field, which is a drop-down selection list.
• The Primary SSH Tunnel field, which is a drop-down selection list.
• The Platform field, which is a drop-down selection list.
• The Description field.
• The Administrator Email field.

In a Multi-Master Cluster, there will be a Make Unique checkbox to the right of the Endpoint Name field which can be used to ensure that the endpoint name is unique across the cluster.
