218_generate_cert_request shows the Generate Certificate Request page. From top to bottom are the following components:

The Generate Certificate Request label and to the right, the Cancel button and the Submit and Download button.

Common Name label showing the automatically populated fully qualified domain name of the Oracle Key Vault server. To its right is a Change button. Following the domain name is the text "Common Name is a fully qualified domain name (FQDN) which matches the host name in the URL used to access this Oracle Key Vault Server.

Either change the host name to FQDN by clicking the Change link above, or add an FQDN as the Subject Alternative Name.
