Cause
The File_Entry_Table contained a duplicate Global_Metadata_Directory metadata structure. Global_Metadata_Directory structures must be unique.
Action
If the ACFS Fixer is currently running, then no further action is needed. The ACFS Fixer will resolve the problem. If the ACFS Checker is running, then run the ACFS Fixer to resolve the problem.
