Cause
The transaction was assigned to the system rollback segment and was trying to get into the PREPARED state. However, Oracle requires a minimum number of transactions to not be in the PREPARED state in the system rollback segment. This transaction could not transition to the PREPARED state, it has been rolled back.
Action
Add a rollback segment and retry the transaction.
