Cause
The script was already being run in a different session or was terminated before status for the script was updated to ERROR or EXECUTED.
Action
Make sure the script is not being run in a parallel session. Then call the RECOVER_OPERATION API in the DBMS_STREAMS_ADM package with the appropriate OPERATION_MODE argument.
