Cause
Current instance was started with pfile, and some Oracle RAC Two-Stage Rolling Updates were rolled back in binary before start. This is not allowed with pfile in use.
Action
If Oracle RAC Two-Stage updates were rolled back, reapply those in binary. If rollback of Oracle RAC Two-Stage updates is required, start instance using shared spfile.
