Service History Questionnaires
Service History questionnaires enable a company to assess the condition of an asset through a series of questions and answers.
The following is a sample of service history questions and answers that could be provided on a questionnaire:
What is the condition of the pipe?
Good. No severe cracks and less than 2 small defects per 100 ft.
Acceptable. No severe cracks and less than 5 small defects per 100 ft.
Needs Attention. No severe cracks but 5 or greater small defects per 100 ft.
Needs Immediate Attention. One or more severe cracks detected.

The bulleted items would appear as radio buttons in the actual questionnaire as answers.

Also note, this question and answer format supports the determination of a rating or score as to the overall condition of the asset.
Questionnaires also support a "fill in the blanks" format. A similar question to that above might appear as follows with “fill in the blanks”:
Assess the condition of the pipe:

Number of severe Cracks: |_​_​_​_​_​_​_​_​_​|
Note:
In this case, each blank would appear as text boxes in the actual questionnaire to capture the answers.
Note:
The “fill in the blanks” format, unlike the radio selection method mentioned above, does not support a rating for the overall condition of the asset.
