Enrollment data is missing
If all enrollment data is not available when you fill out the Enrollment form, or if you missed any required data items on the form, one of the following occurs:
- If your protocol permits enrollment with incomplete enrollment data, an enrollment override page appears. After you obtain approval from an authorized Sponsor representative, you can continue with enrollment.
- If your protocol does not permit enrollment with incomplete enrollment data, the InForm application returns you to the Screening Log after you submit the Enrollment form. The Enrollment Failure column displays “Enrollment Form Incomplete” as the reason for failure. The Enrolled column contains an X, which indicates that the candidate has not been enrolled.
