You can view the following information on the Disclosure Report:

• The data and time of the disclosure.
• The type of Disclosure (for example, Notifications or Clinical Documents).
• The subtype of the disclosure (for example, HL7v2 ADT, SIU, ORN, C62).
• The recipient (in other words, the payer to which the disclosure was made).
• The purpose of use (for example, Payment and Operations).
• The patient's medical record number (MRN).
• The patient's financial identification number (FIN).
• The patient's first name.
• The patient's last name.
• The patient's date of birth.
