Manage Patient Forms

This topic describes how to locate and manage patient forms when managing an encounter.

Assign Forms

To assign a form to a patient, select the appropriate form from the New Form list. The form is displayed. Required forms are displayed as a Needs Attention task.

Print Forms

To print a form:

  1. Select Actions (…) next to the assigned form.
  2. Select Print.
  3. Follow the prompts to print the form and request that the patient sign.

Manually Upload Forms

To manually upload a form:

  1. Select Actions (…) next to the assigned form. Select Upload.
  2. Follow the prompts to scan or upload the document.
  3. Select Save. The form is displayed as completed.

Remove Form Completed in Error

To remove a form added in error:

  1. Select Actions (…) next to the completed form.
  2. Select Remove. The form is removed.

Replace Completed Form

To replace a completed form:

  1. Select Actions (…) next to the completed form.
  2. Select Replace.
  3. Follow the prompts to scan or upload the document.
  4. Select Save. The form is replaced and displayed as completed.

After forms are completed, select Save and continue with the workflow.
