Update Patient Information - Check In

On the Patient Information page, complete the following steps:

  1. Verify the patient information and update as needed.
  2. If you want to add, replace, or delete a patient photo or identity card, see Manage Photos and Identity Cards.
  3. Verify the identity and orientation details, and update as needed.
  4. If any proxy updates are needed, see Work with Proxies.
  5. Verify contact information and update as needed.

    Note:

    The Contact Information prompt is displayed if the patient is older than the patient self-determination age or if the patient is an emancipated minor.
  6. If notifications are enabled and you want to send the patient a text or email to complete new patient registration, select Send self-registration link. When you save, if this option is selected, a text or email is sent to the patient to complete the self registration.
  7. Verify the addresses and update as needed.
  8. Validate the address, if needed. See Validate an Address for more information.
  9. Verify employment details and update as needed.
  10. Select Contine.
