7 Survey Management
The Survey Management module allows you to select existing surveys and create surveys that Patient Portal users can complete based on the appointment type.
Note:
Surveys and questionnaires are intended solely for patient onboarding, registration (patient intake), and gathering of information. Do not use surveys for calculations of diagnosis or for diagnostic purposes.Surveys created in the Survey Management module are displayed in Oracle Health Patient Portal when a Patient Portal user schedules the appointment type associated with the survey. When a patient completes the survey, the survey results are sent to the Oracle Health Millennium Platform database, and clinicians can view responses in the patient's chart.
After you publish a survey, you can create a new draft and make changes. Patients continue to access the previous version of the survey until the new draft is published.

- Draft: Surveys that have been created, but not finalized or shared.
- Collecting: Surveys have been shared and are available to patients when they schedule the applicable appointment type.
- Closed: Surveys have been deactivated and cannot be used.
- Draft: The survey is being built. You can edit surveys in draft status.
- Open: The survey is available to patients. You cannot edit surveys, but respondents can submit answers.
- Closed: The survey has been deactivated. Respondents cannot access the version of the survey. However, it can be reopened if necessary.
- Archived: The survey is no longer available and cannot be reopened.
To access the Survey Management module, from the Administration Console, select Survey Management.
