This topic describes information for the Conditions service.
The Conditions service owns management of patient conditions, which encompass a wide array of clinical concerns, problems, events, situations, issues, or clinical concepts that warrant attention. These conditions often stem from a clinician's evaluation and judgment regarding a particular facet of a patient's health status. Conditions serve as a repository for information on diseases or illnesses discerned through clinical insight applied to pathological findings (diagnoses). They are also used for tracking health issues or situations deemed by a healthcare provider as detrimental or potentially so, warranting further investigation and intervention (problems), or any other health-related matters necessitating continuous oversight, action, or both (health concerns or issues).
The identification of conditions is not solely the domain of clinicians. Patients, their family members, or members of the care team can also express concerns. For instance, a clinician might flag an issue such as anorexia as worrying, even if the patient does not share this concern. Conversely, a patient might experience hair loss, which they consider significant, but might not be deemed critical from a clinical perspective to be included on a practitioner's Problem List.
