Retrieves the Continuity of Care Document (CCD) as binary for the supplied query parameters.

A reference to the encounter that is the subject of the CCD.
Example:encounter=12345

The end of the date range for which the CCD is to be generated. Must be a valid dateTime value with a time component. No prefix is required. If this parameter is not provided, then all records up to the current date are included.

A reference to the patient that is the subject of the CCD.

The start of the date range for which the CCD is to be generated. Must be a valid dateTime value with a time component. No prefix is required. If this parameter is not provided, then all records from the beginning of time are included.
