Kit Order Form [Requested/Fulfilled] notification

This notification is sent when a sponsor user generates an unblinded Drug Order Form for a Quantum-integrated study, including initial, resupply, or manual shipments.

Permissions required to receive the notification

Assign the Receive Notification of Quantum Drug Order Form (Unblinded) permission to sponsor users to receive this notification.

Field descriptions

This table below describes all fields and details in the Kit Order Form [Requested/Fulfilled] notification.

Field	Description
Title	The title is comprised of two distinct elements: The study's ID. For example, a study's ID can be P01-123-A3. The title of the notification, for example, Contents of New Shipment [Shipment Number].
Study	Indicates the study associated with the drug order form that you downloaded.
Mode	Indicates the study mode associated with this notification. Displayed values can be Test, Active, or Training.
Shipment Number	Indicates the shipment number associated with your drug order form.
Date Requested	Indicates the date when the shipment associated with the drug order form has been requested.
(Site/Center) Number	Indicates the site number associated with the requested shipment.
Investigator	Indicates the name of the Principal Investigator, as specified by a site manager when they created the site.
Ship Care Of	Indicates the name of the facility where the shipment is supposed to be delivered, as well as the address of the facility. This facility can be a site or another depot within the study.
Telephone	Indicates the facility's associated phone number, whether it's a depot or site facility.
Fax	Indicates the facility's associated fax number, whether it's a depot or a site facility.
Date Expected at Site	Indicates the projected date of a shipment's arrival at a site.
Number of Study Medication	Indicates the exact number of investigational product kits, specifically for kits, boxes, or bottles.
Special Instructions	Indicates any special instructions specified by an unblinded depot user for other depot users within the study or site users. May contain specific details on how to handle packages or the contents of a shipment.
Kit ID	Indicates the kit's unique identifier.
Kit Description	Indicates the kit's details.
Min. Expiry/Lot	Indicates the minimum expiration for investigational products in an initial or resupply shipments, or the lot details for manual shipments.
Quantity Requested	Indicates the number of kits requested.
Kit / Lot Numbers	Indicates the number of kits fulfilled in the shipment. Note: This field contains data only for the Kit Order Form [Fulfilled] notification.
Signature	This is the field that a site or depot user is supposed to sign upon receiving and registering a shipment.
Date	This is the field that a site or depot user is supposed to fill-in with the date of the shipment reception.
