Map lab form questions

You can use either a Lab Form or a one-section repeating form to collect lab data in a study that supports Electronic Health Record (EHR) data import.

Unlike a Lab Form, a one-section repeating form does not automatically include Sample Collection Date, Fasting, Lab Test, Lab Result, Lab Unit, and lab normal reference ranges. Instead, you must incorporate these fields and values into the form design.

Before creating a lab form

• After a study version is moved from the Draft container, adding, updating, or removing EHR mappings is permitted only as part of a study version change. Advanced Study Versioning (ASV) does not support EHR mapping changes.
• If you hide the Collection Date Time field, EHR lab import is blocked unless the importing user has permission to view hidden fields through data classification settings.
• When Allow for Multiple Instances is enabled on the Lab Form (not a one-section repeating form), EHR lab results from different collection dates can be imported into separate lab form instances. If multiple instances are not allowed, only one collection-date group can be imported at a time.

To create a lab form

1. Open a study's design and then open the Draft study version.
2. In the Forms tab, select 1 Section or Lab Form from the Create Form drop-down.

   For a Lab Form, the relevant questions are added to the form automatically. For a one-section repeating form, you must add the questions manually.

3. For a one-section repeating form, select Add to add questions.
4. Expand the Advanced sidebar for each question to be mapped for EHR data import.

   A Lab Form only supports mappings for Lab Test Name, Lab Result, and Lab Date of Collection.

   You can map the Lab Test Name field on multiple lab forms (Lab Form and 1-section) within a study. Each Lab Form must use a unique set of Lab Test values from the EHR-Lab Test code list. Do not assign the same Lab Test value to more than one mapped Lab Form.

5. From the EHR Mapping drop-down, select the mapping option that corresponds to the question. For example, select Labs > Lab Test Name to map the Lab Test Name question.
   The Add Questions dialog opens and displays additional pre-mapped questions. For a one-section repeating form, the system adds the questions to the form and automatically maps them. For a Lab Form, the system automatically maps the corresponding existing questions.
6. Select the relevant questions, then select Add or select Not Now if you plan to add these questions manually at a later time.
   If you select Add, the questions are added and mapped to the form and marked EHR Mapped.
7. After mapping Lab Test name, select Code List to apply the EHR-Lab Test code list. For more information, see About Electronic Health Record (EHR) code lists.
