Subject record transfer affidavit
The subject record transfer feature of the Oracle InForm application enables users to transfer a subject's clinical record to a new site. This feature is useful in situations where:
- A subject moves to a different location.
- A subject moves seasonally to another address.
- An investigator leaves a study and closes down a site.
When a user transfers a subject to another site, the Oracle InForm application displays an affidavit that provides guidance on the responsibility of the administrative user regarding moving a subject from one site to another.
