This view contains association and disassociation history data from associated forms.

Column Name (Key indicator)	Column Description
TRANSACTIONID	Transaction identifier from which the form association was created or modified.
SUBJECTID	Numeric identifier for the patient associated with the forms.
SITEID	Numeric identifier for the site associated with the forms.
VISITID	Numeric identifier for the visit in which the associated forms exist.
VISITINDEX	Instance identifier for the visit in which the associated forms exist.
STATUS	Status of the form association between two form instances a given row pertains to at a given point in time: 0: Association created, that is active form association at this time point. 1: Association removed: pre-existing form association removed at this time point.
FORMASSOCID	Numeric identifier for the form association instance.
PAGEASSOCREVISIONNUMBER	Instance identifier for the revision of a given form association.
FORMID	Numeric identifier for one of the associated forms.
FORMINDEX	Form instance identifier for one of the associated forms.
ASSOCVISITID	Numeric identifier for the visit in which the associated forms exists.
ASSOCVISITINDEX	Visit instance identifier for the visit in which the associated forms exists.
ASSOCFORMID	Numeric identifier for one of the associated forms.
ASSOCFORMINDEX	Form instance identifier for one of the associated forms.
CREATEDDATETIME	Date and time when the form association instance was created.
CREATEDBYUSERID	User ID of the user whose action resulted in the given view row being created.
MODIFIEDDATETIME	Date and time when the data in the view row was most recently updated.
MODIFIEDBYUSERID	User ID of the user whose action resulted in the given view row being created.
REASON	Reason for the form association instance creation or modification.
