Audit Trail Report

This report shows the fields for which data was entered, modified, or cleared within a selected date range.

Column descriptions

Column	Description
Study Name	Data type: String Description: Name associated with the study, as specified in the study design code. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Site Mnemonic	Data type: String Description: Abbreviation used to identify a site, as entered in the InForm Administration interface. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Subject Number	Data type: String Description: Number that identifies the subject. This number may be changed after the subject is enrolled, if the study is configured to require a unique subject ID for a study or site. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Visit Mnemonic	Data type: String Description: Abbreviation of the visit title, as specified in study design code. For CRF visits, this name appears at the top of a collection of forms in the visit timeline. The visit mnemonic may not be a unique identifier. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Visit Index	Data type: Numeric Description: Value that identifies the instance of the visit in which data was entered for the form. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Form Mnemonic	Data type: String Description: Short name or abbreviation for the form, as specified in the study design code. This name appears on the Visit Navigation tab and is used to access a specific form in the InForm application. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Form Index	Data type: Numeric Description: For repeating forms, a numeric ordinal value that identifies the instance of the form. For non-repeating forms, the value is 1. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Itemset Index	Data type: Numeric Description: Value that identifies an instance of an itemset. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Item Question	Data type: String Description: Text of the question or prompt that instructs the user to enter specific clinical data. Items that have never contained data will not appear on reports that pair Item Question with Form Name or Form Mnemonic. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Changed Control RefName	Data type: String Description: Name of the modified field. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Audit Order	Data type: Numeric Description: Value that indicates the audit order for item changes from the earliest to the latest. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Initial Entry or Modification	Data type: String Description: Text that explains whether the clinical data is an initial entry, a modification, or a deletion. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Entered Reason	Data type: String Description: Text that explains the reason for the most recent clinical data change (if any) for the item. Text appears here only when the clinical data for the item has been changed after initial entry. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Current Value	Data type: String Description: Current value of the item. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Current Value Date/Time	Data type: String Description: Current date/time value for the item. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Previous Value	Data type: String Description: Value of the item before it was modified. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Previous Value Date/Time	Data type: String Description: Date/time value of the item before it was modified. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Rights Group	Data type: String Description: Rights group to which the user who entered or modified the data belongs. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Username	Data type: String Description: User name of the user who entered or modified the data. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
User Type	Data type: String Description: User type. For example, Sponsor. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
