This screen has two groups of fields, Node Manager Type (top) and Node Manager Credentials (bottom). The Node Manager Type section has three radio buttons, from top-to-bottom, Per Domain Default Location, Per Domain Custom Location, and Manual Node Manager Setup. There is a Node Manager Home field under these buttons. There is a Browse button to the right of the field. The field is not editable if the Per Domain Default Location button is selected.

The Node Manager Credentials section contains three fields, from top-to-bottom, Username, Password, and Confirm Password.
