To get feedback about the learning material or the instructor, you can add an
evaluation.
Here's what to do

On the offering details page, click the Activities tab.

On the Overview section, Add menu, select Add
Evaluation.

In the Evaluation section, the last section on the Activities tab, specify whether
to use the default evaluation or another evaluation. The name of the default
evaluation appears as read-only information in the Evaluation field. To help you
decide, you can preview the evaluation. If you select another evaluation, you can
also preview it.
Tip: If you use the default evaluation and it gets changed later, anyone
completing the evaluation after the change gets the new evaluation. It doesn’t
matter if you change the offering definition. If you select a different evaluation
and the default evaluation changes, learners continue to get the selected
evaluation.

If using an evaluation other than the default, you can specify whether learners
need to complete the evaluation before Oracle Learning considers the offering
complete. If you use the default evaluation, the setting on the Configure Evaluations
page applies. You can't override it.

Click Save and Close.
