Forms thread name addition.

Parameters:

pRequest - current DynamoHttpServletRequest request.

pStringBuffer - contains formed name addition.

service

                    DynamoHttpServletResponse pResponse)
             throws java.io.IOException,
                    javax.servlet.ServletException

For the life of the request names the Thread running with the original name, appending the hostname or ip address of the request and session id, if they are available in the request. At the end of the request the original name is put back.
