You must report OSHA information for each establishment in the organization that meets OSHA reporting criteria. To do this reporting, you must identify each employee who works in an OSHA reportable establishment. You identify the establishment in which an employee works by using supplemental data.

Before you can identify the establishment in which an employee works, you must define the valid search types for establishments in UDC 08H/ES, then add the establishments to the address book with the defined search type. You need to enter effective dates in the supplemental data for OSHA reporting. After you set up the establishment data type, you can attach establishment information to each employee who works in an OSHA reportable establishment.

An establishment is any single location where the organization conducts business or performs services. When distinctly separate activities are performed at a single physical location, each activity is a separate establishment. An establishment can be a field activity or an office, installation, or facility. You create groups of establishments to satisfy requirements for OSHA reports and the Survey of Occupational Injuries and Illnesses.

When you set up establishment information, you can include conditions that might have affected the establishment during a given year. You can include conditions such as natural disasters, layoffs, strikes, changes in working schedules, or any other event that might affect OSHA reporting information. You can enter multiple conditions for a given year and establishment.

Establishment information is required for the Survey of Occupational Injuries and Illnesses. The conditions that you include might occur even if the establishment had no injuries or illnesses to report.

You can create a grouping of establishments that you might want to use repeatedly for reporting information. Creating an establishment grouping prevents you from having to enter each establishment that you want to include in the report every time that you run a report.

 

Note. Each entry that you make in the detail area of the Establishment Form (W086011A) must be a unique combination of calendar year, establishment, and establishment conditions.

Access the Establishment form.

Calendar Year

Enter the 4 digit calendar year.

Establishment

Enter the establishment where the accident or illness occurred.

Establishment Condition

Enter a code that indicates whether any conditions affected the establishment in a given year. The codes and definitions that the system provides with the software should not be changed.

The values for Establishment Condition field are stored in UDC (defined code) table 05/CO. You can add conditions to this table, as needed.
