Middle Name | Middle name of the user. |

Custom Homepage URL | Domain-specific URL for a custom Dashboard. | If non-applicable, leave blank. |
Role | Role to which the user is assigned. | Required. To work in the application and receive notifications, the user must be assigned to at least one role. |
Language | Language for all text in the application. | If the selected language is supported, all pages appear in the selected language for the user when you submit the settings on this page. |
ZIP code (postal code) | ZIP code or postal code of residence. | Required. |
Country | Country of residence. |

Receive Email | Whether to send email notifications to the user or patient. |

Email Address | Email address for notifications. | Maximum length: 40 characters. Must be unique. The ending must be standard for an email address, such as .com, .org, or .edu. |
User Name | User name for signing into the application. | Required. The user name cannot be an email address. The user name can contain underscore characters. |

All Sites | When Yes is selected, the user is assigned to all sites, including sites that you add in the future. |

Available Sites | Sites to which you can assign the user. | To work in the application, the user must be assigned to at least one site. |
Selected Sites | Sites to which the user is assigned. |

Note: If you create a new role type using the Manage Roles and Rights link in the Administration module, that new role type appears in the Roles list and can be selected. When you create a new user using the Manage Users link, the new role type is available to assign to the user.
