  Fields on the Manage Sites page

  Field	Description	Note
  Site Information
  Site Name	Name of the site.	Required. The application prevents you from specifying a site name that differs from an existing site name only by capitalization.
  Organization	Sponsor organization name.
  Number	Identifier assigned for arm management.	Required.
  ZIP code (postal code)	Site ZIP code or postal code.	Required.
  PO Box	Post office box number.
  Target Number of Patients	Desired number of patients to be assigned to the site.
  Maximum Number of Patients	Maximum number of patients who can be assigned to the site.	Required. Can be larger than the target number.
  Limit Patients	Whether to limit the number of patients who can be assigned to the site.
  Enable site for ePro	Whether the site allows ePRO forms.	Required.
  Active	Whether the site is active.	Required. All sites appear in the Available Sites list, and you can assign patients to them. For more information, see Making a site active or inactive.
  Country	Country where the site is located.
  Time Zone	Time zone in which the site is located.
  State or Province	State or province where the site is located.
  City	City where the site is located.
  Street	Street number and name where the site is located.
  Building	Building name or number where the site is located.
  Room	Room number within the building.
  Address	Street address of the site.
  Phone	Phone number for contacting a site representative.	Required. Up to 20 digits.
  Date Format Name Sort Number Format Currency Format	Date format, how names are sorted, number format, and currency type.	You can also specify these settings at the: Study level—On the Manage Localization Settings page. User level—On the Create User and User Profile pages.
  List of Available ICDs	Informed Consent documents that may be associated with the site.
  Selected ICDs	Informed Consent documents associated with the site.
  Available Destination Sites	All sites available to be associated with the originating site for patient transfers.
  Selected Destination Sites	All sites associated with the originating site as destination sites for patient transfers.
  List of sites
  Edit link	View and edit information for the selected site.	Appears only if you can edit sites.
  Change Users link	Change which users are assigned to the site.	Appears only if you can edit user roles and rights.
  Site Name	Name of the site.
  Organization	Sponsor organization name.
  Actual Number of Patients	Number of patients assigned to the site.
  User Count	Number of users assigned to the site.
  Address	Street address of the site.
  Phone	Phone number of a site representative.
  ZIP code (postal code)	Site ZIP code or postal code.
  Country	Country where the site is located.
  Time Zone	Time zone in which the site is located.	You can also specify this setting at the: User level—On the Create User and User Profile pages.
