Patient Summary page

Only users with the appropriate rights can view and use certain buttons.

  Fields in the Patient Summary page

  Field	Icon	Description	Notes
  Tabs
  Patient Summary tab		Displays the Patient Summary page for the patient.
  Patient Forms tab		Displays the Patient Forms page for the patient.
  Patient ICD History tab		Displays the Patient ICD History for the patient.
  Fields
  Checkbox		Use to select the patient, a visit, a form, or an item.
  Patient ID		Unique identifier for the patient.
  Patient Initials		Initials of the patient.
  Status		Visit, form, item, and query status at the patient level.	The application applies the Not Started, In-Progress, Complete, and Open with Queries statuses to repeating sections. A Not Started status icon applied to a repeating section means that the repeating section has not been submitted.
  Columns
  Visits		Click the Visit name to link to the Visit landing page for the visit or the first form. Click the arrow to expand a visit and display the forms within the visit and any repeating sections within a form. Click the arrow next to a form to display the items on the form.	Repeating sections within forms appear in descending order by creation date. The newest instance appears first.
  Actual		Actual visit date.
  Open		Number of open queries on patient forms.
  Answered		Number of answered queries.
  Approved		Number of approved queries.
  Closed		Number of closed queries.
  Query Status		Visit, form, item, and query statuses.
  System Status		Status of automatically issued queries.
  Manual Actions		Status of manually issued queries.
  Status and Review icons	Icon
  Create Query		Create a query about the patient, visit, or form.
  Response Query		Respond to a query.
  Modify Query		Modify a query.
  Approve Query		Approve a query.
  Re-issue as Open		Re-issue a query.
  Close Query		Close a query.
  Unlock		Unlock the patient, visit, or form.
  Lock		Lock the patient, visit, or form.	No further modification to the data is permitted.
  Unmark DM Reviewed		Unmark the patient, visit, or form as DM Reviewed.	Changing the data automatically activates the Unmark status.
  Mark DM Reviewed		Mark the patient, visit, or form as DM Reviewed.	A data manager has reviewed the data entries.
  Remove Remote Review		Unmark the patient, visit, or form as Remote Review.	Changing the data automatically activates the Unmark status.
  Mark for Remote Review		Mark the patient, visit, or form as Remote Review.	The form or visit has been reviewed remotely and found to be complete. The data was not checked against source documents.
  Unmark SDV		Unmark as SDV (Source Document Verification) reviewed.	Changing the data automatically activates the Unmark status.
  Mark SDV		Mark as SDV (Source Document Verification) reviewed.	The data on the form has been reviewed against the source data.
  Unfreeze		Unfreeze the patient, visit, or form.
  Freeze		Freeze the patient, visit, or form.	The data has been reviewed and cannot be modified at this time; however, you can open queries.
  Left blank		Intentionally left blank.	The sponsor has chosen to skip this visit, form, or item.
  Unsign		Unsign the selected patient, visit, or form.	A site user, typically an investigator or clinical research coordinator, performs this action. Changing the data automatically activates the Unsign status.
  Sign		Sign the selected patient, visit, or form.	The OutcomeLogix application automatically requests authentication of the user.
  Signed at parent level		An object at a higher level in the object hierarchy has been signed (for example, the visit in which a form appears has been signed).	The design of your study determines whether a visit, form, or item that is signed at a higher level in the object hierarchy can be considered signed itself.
