Patient Forms page

The following task uses this page:

• Reviewing patient forms.

  Fields in the Patient Diary page

  Field	Description
  Patient Forms Review
  Patient ID	Unique identifier for the patient assigned by the OutcomeLogix application.
  Year button	Displays the patient forms for the current year, month by month. Months are column headings, and each column contains the forms completed that month. Click a form link to display the completed form. You cannot edit the form. Use the Year drop-down list to navigate to a different year.
  Month button	Displays the calendar for the current month. The forms completed appear in the boxes for each day. Click a form link to display the completed form. You cannot edit the form. Use the Year and Month drop-down lists to navigate to another month.
  Week button	Displays the patient forms completed day by day for the current week. Click a form link to display the completed form. You cannot edit the form. Use the Year and Month drop-down lists to navigate to another month. Use the << and >> arrows to navigate to a different week.
  Visit	The visits for which there are forms to review.
  View link	Click the link to display the forms within a visit, the date entered, and the status of the form.
  Back button	Return to the Patient Forms page.
  Patient Summary tab	Return to the Patient Summary page.
