Site Number

Identification number assigned to the site.

Site Name

Full name of the site.

Patient ID

Unique identifier for the patient.

Visit Name

Name of the visit in which the item exists.

Visit Index

Visit index for the visit in which the item exists.

Form Name

Name of the form on which the item exists.

Form Index

Form index for the form on which the item exists.

Item Name

Variable name for the item.

Initial Entry

Yes or No: Indicating whether the original value for the item was the first value entered for the item, or whether it was a modified version of the first value entered.

Previous Value

Previous value for the item.

Current Value

Current value for the item.

Change Date (GMT)

Date and time when the item value was changed.

Reason For Change

Reason for changing the item value, as entered by the user who modified the item.

Role

Role assigned to the user who modified the item.

Username

User name of the user who modified the item.
